The district is committed to
providing students comprehensive education and support that
meets their health and wellness needs. This commitment is
reflected in its school nurse offices, its wellness policy,
health and physical education curricula, and its adherence to state
and federal health regulations and guidelines.

Child Passenger Restraints Are Not An
Option.
They Are The Law!
Effective November 24, 2009:
•All children must be restrained in an appropriate child
restraint system while riding in a motor vehicle, until they
reach their 8th birthday.
•An appropriate child restraint system is one that meets the
child's size and weight recommended by the manufacturer.
•The safety belt in your vehicle is not designed for children. A
booster seat raises your child up so that the safety belt fits
your child correctly and provides better protection.

DISTRICT WELLNESS
POLICY
Port Jervis has an active Wellness Advisory Committee that
developed and continuously evaluates the district's
Wellness Policy.
ABSENCES & ATTENDANCE
If your child is absent from school
(K-12), it is important for the parent/legal guardian to call
your child’s school on the day of the absence. Please send in a
written note when your child returns to school. The note must
include the date(s) of absence, the reason, and the
parent/guardian’s signature. If your child is absent for more
than a few days, the school nurse may request a written note
from your child’s doctor.
When to keep your child home
Children should be kept home if they
show signs of a cold, parasitic or other infection. After a
fever, a child should not return to school until the temperature
has been normal for at least 24 hours.

SCREENINGS
Port Jervis Health Services program supports
student academic success by promoting health in the school
setting. One way that we provide care for your student is by
performing health screenings mandated by the State of New York.
SCHOOL HEALTH SCREENINGS
IMMUNIZATIONS
New York State law requires all
students to be immunized against polio, rubella, hepatitis B,
diphtheria, mumps, and measles. Proof of immunization for new
students must be presented for admission.
Pre-K children born on or after Jan. 1, 2000, and sixth graders
born on or after Jan. 1, 1994, must be immunized for varicella
unless parents can present proof of vaccination or a doctor's
note verifying the date the child had the chicken pox disease.
Students entering sixth grade, who are 11 years or older, must
receive the Tdap vaccine.
Free immunizations available
All required shots are available from family doctors or at the
free Orange County immunization clinic.

MEDICATIONS
IN SCHOOL
In order for a school nurse to give a
student medications during school hours, the
Medication
Release Form
must be
signed by the student's
parents and
prescribing physician.

FLUORIDE DISTRIBUTION
The district, in cooperation with the New
York State Department of Health, is offering elementary students
(grades K-6) a free supplemental fluoride tablet program to help
reduce the risk of tooth decay. The Center for Disease Control
and Prevention recommends fluoride supplements for children who
live in communities with less than optimum levels of fluoride in
their drinking water. The fluoride tablets
are distributed daily to students in grades K-6 under direct
supervision.
SPORTS
PHYSICALS
Students who do not
have a physical will be considered ineligible for sports until they
are cleared by the nursing staff. Athletes
need to bring the required sports forms, signed by their
parents, with them to the physical.

can be downloaded below or obtained in advance from the Middle School or High School
Health Offices. Questions can be directed to the PJHS Health Office, 858-3125, or the Athletic
Office, 858-3126.
MRSA
INFECTIONS
MRSA is
a type of Staphylococcus aureus bacteria normally carried on the
skin and in the nose of healthy people. This infection
commonly causes boils and soft tissue infection. If your child
is displaying symptoms or if you have questions, please contact your family physician.
Please report a diagnosed or suspected infectious
condition to the school nurse, as well as your child's coach
(if he/she is an athlete).
